The Thresholds Centers for Mental Wellbeing provide a wide range of mental health and substance use treatment services to their immediate surrounding communities. Thresholds has four Centers for Mental Wellbeing, with locations in McHenry County, Kankakee County, Chicago’s Ravenswood neighborhood, and the South Side of Chicago.
The Centers for Mental Wellbeing follow the federal Substance Abuse and Mental Health Services Administration (SAMHSA) model of care delivery for Certified Community Behavioral Health Clinics (CCBHC).
Thresholds’ Centers for Mental Wellbeing serve as a “one-stop shop” for mental health, substance use treatment and other health care services in their communities. The CCBHC model ensures access to 24/7/365 crisis care, care coordination with hospitals, law enforcement, and schools, and access to high-quality services that include medication-assisted treatment (MAT) to help stop the overdose crisis, among other capabilities.
Thresholds was awarded two grants totaling $2 million, renewable for up to 4 years, by SAMHSA in 2022, allowing us to launch both Centers for Mental Wellbeing. This new care delivery model opens up services to populations that we have previously been unable to serve, including children and families. The Center for Mental Wellbeing – Ravenswood is among the first CCBHCs in Chicago. Thresholds was awarded an addition two grants in 2023 for future sites in Kankakee County and the South Side of Chicago, launching in 2024.
Thresholds also partners with Aunt Martha’s Health and Wellness and Rosecrance in McHenry County and Tapestry 360 Health in Chicago to provide primary care and substance use treatment to youth and adults through the CCBHC model.
Our Centers for Mental Wellbeing are committed to providing care regardless of ability to pay, and care for those who are underserved, low income, are insured, uninsured, or on Medicaid. We offer a sliding scale program that provides discounted services to individuals based on family size and income.
