Thresholds to Launch a New Primary Care Health Center
We are so excited to share the news that Thresholds is launching a new, independent entity called Thresholds Health later this summer. Thresholds Health will be a full-service primary care health center that serves both existing Thresholds clients and members of communities on the west side of Chicago.The first Thresholds Health clinic will be located in Chicago’s Austin community area. We know that the integration of primary, mental health, and substance use care is a proven approach to address complex healthcare needs. Thresholds will continue to work with our valued current primary health care partners, including Heartland Health Centers, Howard Brown Health, and Aunt Martha’s.Thresholds Health is launching a search for a dynamic leader to launch, lead, and grow this new primary care health center. The CEO of Thresholds Health will oversee all aspects of this start-up enterprise, as well as its application to become a Federally Qualified Health Center Look Alike (FQHC LAL).
